Lifestyle Support Programme
The lifestyle support programme has been designed to target people already living with diabetes, coronary heart disease or who are at risk of developing cardio vascular disease in the future but may not be aware of their risk currently.

Access to the programme is through your GP, certain criteria must be met for you to be referred onto the programme. If you are referred a lifestyle coach will develop a personal health action plan. Please discuss with your GP or practice nurse if you feel you are eligible.
